Research Elective Approval Forms

Instructions

1. If you haven’t already activated your UCSF DocuSign account, please follow these instructions:
   • Login to MyAccess: https://myaccess.ucsf.edu
   • Scroll down to “DocuSign – UCSF” and click on the link to DocuSign. This will initiate creating your account if you don’t already have one (the account creation takes a minute or so).
   • Once signed in, you will see a status box, as well as help resources should you want more info about DocuSign.
2. Select the link below for the corresponding department's research elective approval form. If you have two supervisors for your research project, use the link in the second box below; your form will be routed to both supervisors for approval. 
3. Follow the instructions for completing the form. It will be sent electronically the listed Instructor of Record.
4. When the course is approved, you will receive an email confirmation.
5. Additional information for international research work:
   • You must email Michael Lipnick if you are doing international research work before you submit the Research Elective Approval form
   • The International Emergency Packet and the International Student Travel Approval form: These forms are required of all students requesting credit or funding for time spent abroad doing clerkships or research work.
     • The International Emergency Packet must be completed.
     • The Student International Travel form must be completed, approved, and signed.
     • Both forms (combined into one document) must be attached to your Research Elective Approval form (150.01/105.02)
