"Eczema coxsackium" and unusual cutaneous findings in an enterovirus outbreak.

OBJECTIVE
To characterize the atypical cutaneous presentations in the coxsackievirus A6 (CVA6)-associated North American enterovirus outbreak of 2011-2012.
METHODS
We performed a retrospective case series of pediatric patients who presented with atypical cases of hand, foot, and mouth disease (HFMD) from July 2011 to June 2012 at 7 academic pediatric dermatology centers. Patients were included if they tested positive for CVA6 or if they met clinical criteria for atypical HFMD (an enanthem or exanthem characteristic of HFMD with unusual morphology or extent of cutaneous findings). We collected demographic, epidemiologic, and clinical data including history of skin conditions, morphology and extent of exanthem, systemic symptoms, and diagnostic test results.
RESULTS
Eighty patients were included in this study (median age 1.5 years, range 4 months-16 years). Seventeen patients were CVA6-positive, and 63 met clinical inclusion criteria. Ninety-nine percent of patients exhibited a vesiculobullous and erosive eruption; 61% of patients had rash involving >10% body surface area. The exanthem had a perioral, extremity, and truncal distribution in addition to involving classic HFMD areas such as palms, soles, and buttocks. In 55% of patients, the eruption was accentuated in areas of eczematous dermatitis, termed "eczema coxsackium." Other morphologies included Gianotti-Crosti-like (37%), petechial/purpuric (17%) eruptions, and delayed onychomadesis and palm and sole desquamation. There were no patients with serious systemic complications.
CONCLUSIONS
The CVA6-associated enterovirus outbreak was responsible for an exanthem potentially more widespread, severe, and varied than classic HFMD that could be confused with bullous impetigo, eczema herpeticum, vasculitis, and primary immunobullous disease.

STUDY OBJECTIVES
To determine the patterns of antidepressant and atypical antipsychotic use in a veteran population with depression, and to determine if an association exists between specific antidepressant classes and atypical antipsychotics and the occurrence of cardiovascular events and all-cause mortality.
DESIGN
Retrospective, cross-sectional study.
SETTING
Primary care clinic at a Veterans Affairs hospital.
PATIENTS
A total of 1136 patients diagnosed with depression who were receiving antidepressant monotherapy (664 patients) or no antidepressant therapy (472 patients [controls]) between June 2009 and December 2010.
MEASUREMENTS AND MAIN RESULTS
Data on patient demographics, disease diagnoses, laboratory data, and drug therapy profiles were collected through medical record review. Of the 1136 patients, the mean patient age was 61 years, 90% were men, and 77% were smokers. Mean body mass index was 30.4 kg/m(2) , blood pressure 126/73 mm Hg, hemoglobin A1c 6%, low-density lipoprotein cholesterol level 106.7 mg/dl, and Framingham score 17. Patients receiving antidepressant monotherapy were grouped according to antidepressant class; selective serotonin reuptake inhibitors (SSRIs) were most common. Concomitant use of atypical antipsychotics was more common with the serotonin-norepinephrine reuptake inhibitor (venlafaxine), SSRI, and serotonin receptor antagonist (trazodone) classes (p=0.0067). After adjusting for demographics, concomitant drugs, and comorbidities, SSRI use was significantly associated with lower all-cause mortality (odds ratio [OR] 0.37, 95% confidence interval [CI] 0.19-0.71, p=0.0028). Notably, noradrenergic and specific serotonergic antidepressant (mirtazapine) use was significantly associated with higher prevalence of heart failure (OR 3.26, 95% CI 1.029-10.38, p=0.0445). Use of atypical antipsychotics was significantly associated with a higher prevalence of cerebrovascular events (OR 2.23, 95% CI 1.29-3.83, p=0.0036) and all-cause mortality (OR 2.05, 95% CI 1.03-4.1, p=0.04).
CONCLUSION
Our results favor treatment of depression with SSRIs among patients at increased cardiovascular risk due to the potential mortality benefit of this class of drugs. Atypical antipsychotics should be used with caution in the elderly population. Mirtazapine use in patients with heart failure and depression deserves further investigation.

OBJECTIVE
To describe the epidemiology of genitourinary (GU) injury from toilets that present to USA Emergency rooms (ERs).
MATERIALS AND METHODS
The National Electronic Injury Surveillance System (NEISS) is a stratified probability sample of hospital ER visits for USA consumer product-related injuries. NEISS was used to estimate total toilet- and toilet seat-related GU injury for the years 2002-2010, as well as to describe demographics and injury characteristics. Analyses were performed using strata, primary sampling units and sample weights to accommodate the complex sample survey design. Data are reported as national estimates with 95% confidence intervals (CIs) provided.
RESULTS
In all, 13 175 (95% CI 10 185-16 165) GU injuries related to toilets presented to ERs during 2002-2010. The most common mechanism involved crush from accidental fall of toilet seat, described in 9011 (68.4%, 95% CI 6907-11 115) cases. Most crush injuries were isolated to the penis (98.1%). Of crush injuries, 81.7% occurred in children aged 2-3 years and 99.3% occurred in the home. Crush injuries increased over the period 2002-2010 (P = 0.017) by ≈100 per year, ending with an estimated 1707 (95% CI 1011-2402) by 2010. Most patients who sustained toilet- and toilet seat-related GU injuries were treated in the ER and then discharged.
CONCLUSION
While penile crush injury related to a toilet seat is an uncommon mechanism of urological injury in children, the number of incidents appears to be rising. These findings support educational efforts and interventions, such as exchange of heavy toilet seats with slow-close toilet seat technology.

BACKGROUND
Mucoepidermoid carcinoma (MEC) is an uncommon malignancy. To the best of our knowledge, this is the largest study investigating disease-specific survival (DSS) of parotid MEC and the first population-level study of the distribution of nodal metastases.
METHODS
Patients with MEC of the parotid gland were identified in the Surveillance, Epidemiology, and End Results (SEER) database (1988-2009).
RESULTS
We identified 2400 patients with MEC: 522 low grade, 1137 intermediate grade, and 741 high grade. Five-year DSS rates for low-grade, intermediate-grade, and high-grade MEC were 98.8%, 97.4%, and 67.0%, respectively (p .001). Negative prognostic factors included high grade, increasing patient age, and tumor size, extraparenchymal extension, nodal metastases, and distant metastases. High-grade MEC was more likely to have lymph node metastases in levels I to III (34.0%) than low-grade (3.3%) and intermediate-grade MEC (8.1%; p .001).
CONCLUSION
Grade influences the prognosis and distribution of nodal metastases. Results indicate that management guidelines should vary based on grade.

This article reviews recent publications evaluating the current epidemiology of urologic trauma. The authors briefly explain databases that have been recently used to study this patient population and then discuss each genitourinary organ individually, utilizing the most relevant and up-to-date information published for each one. The conclusion of the article briefly discusses possible future research and development areas pertaining to the topic.

The Superior Articular Process as an External Landmark for Determining the Transverse Plane Angulation of Thoracic Pedicles.

STUDY DESIGN
Computed tomographic (CT) study of thoracic spine pedicles.
OBJECTIVE
To analyze the usefulness of the superior articular process (SAP) as an external landmark for determining the transverse plane angulation of thoracic pedicles.
SUMMARY OF BACKGROUND DATA
The use of thoracic pedicle screws has become commonplace. Although most authors report them to be safe, their use poses a risk to neurovascular structures. Previous studies have provided useful information regarding thoracic pedicle anatomy, but this information is difficult to apply intra-operatively. To avoid neurovascular injury, it is important to determine the correct transverse plane angulation of screw insertion.
METHODS
Two separate investigators reviewed thoracic spine CT scans of 53 patients, 26 years of age or younger. Measurements were taken of the angular relationship of the pedicle and the SAP of T4-T11. A 90° angle was subtended from a line parallel to the SAP with a starting point at the midpoint of the lateral half of the SAP. Measurements were then adjusted laterally for medial breeches and medially for lateral breeches, to align the trajectory down the middle of the pedicle. The degree of correction was recorded. Each investigator made 3 sets of measurements. We calculated kappa values to assess intra-observer/interobserver agreement.
RESULTS
Of the 4,008 measurements, 95.2% were contained within bone, leaving 4.8% pedicle violations. The average correction made for medial and lateral breeches was 6.3% and 6.7%, respectively. The first rater had 92.6% agreement, and an intra-observer kappa value of 0.57. The second rater had a 95.3% agreement and an intra-observer kappa value of 0.40.
CONCLUSIONS
The results support the hypothesis that the SAP can be a useful external landmark for determining the transverse plane angulation of thoracic pedicle screw insertion.

There is a paucity of research demonstrating how HIV-funded services in Africa have improved equity and access to non-HIV services for both HIV-infected and uninfected patients. In this short communication, we describe the impact of an airborne outreach program to provide HIV services to high-HIV burden health facilities in rural Botswana. The analysis demonstrates how this HIV-funded program enhanced access to essential subspecialist services at several rural health facilities across Botswana.

OBJECTIVE
The human major histocompatibility complex class I chain-related gene A (MICA) controls the immune process by balancing activities of natural killer cells, γδ T cells and αβ CD8 T cells, and immunosuppressive CD4 T cells. MICA is located near HLA-B on chromosome 6. Recent genomewide association studies indicate that genes most strongly linked to ankylosing spondylitis (AS) susceptibility come from the region containing HLA-B and MICA. While HLA-B27 is a well-known risk genetic marker for AS, the potential effect of linkage disequilibrium (LD) shields any associations of genes around HLA-B with AS. The aim of this study was to investigate a novel independent genetic association of MICA to AS.
METHODS
We examined 1543 AS patients and 1539 controls from two ethnic populations by sequencing MICA and genotyping HLA-B alleles. Initially, 1070 AS patients and 1003 controls of European ancestry were used as a discovery cohort, followed by a confirmation cohort of 473 Han Chinese AS patients and 536 controls. We performed a stratified analysis based on HLA-B27 carrier status. We also conducted logistic regression with a formal interaction term.
RESULTS
Sequencing of MICA identified that MICA*007:01 is a significant risk allele for AS in both Caucasian and Han Chinese populations, and that MICA*019 is a major risk allele in Chinese AS patients. Conditional analysis of MICA alleles on HLA-B27 that unshielded LD effect confirmed associations of the MICA alleles with AS.
CONCLUSIONS
Parallel with HLA-B27, MICA confers strong susceptibility to AS in US white and Han Chinese populations.
