The spread of coronavirus disease (COVID-19) infection across the world accelerated the adoption of social media as the platform of choice for real-time dissemination of medical information. Though this allowed useful clinical anecdotes and links to the latest articles related to COVID-19 to quickly circulate, the broad use of social media also highlighted the power of platforms such as Twitter to spread misinformation. Trainees in medicine have important perspectives to share on social media but may be reluctant to do so for a variety of reasons. There is a need to provide guidance on how to safely engage with social media as well as move the conversation forward in a meaningful way. In this manuscript, we suggest a stepwise approach for trainee social media engagement that integrates the modified Bloom's Taxonomy for social media with Aristotle's principles of rhetoric. This provides trainees with guidance on making ethical, logical, and persuasive cases on social media when creating, consuming, promoting, and discussing content produced by themselves or others.

Predicting Self-Rated Health Across the Life Course: Health Equity Insights from Machine Learning Models.

BACKGROUND
Self-rated health is a strong predictor of mortality and morbidity. Machine learning techniques may provide insights into which of the multifaceted contributors to self-rated health are key drivers in diverse groups.
OBJECTIVE
We used machine learning algorithms to predict self-rated health in diverse groups in the Behavioral Risk Factor Surveillance System (BRFSS), to understand how machine learning algorithms might be used explicitly to examine drivers of self-rated health in diverse populations.
DESIGN
We applied three common machine learning algorithms to predict self-rated health in the 2017 BRFSS survey, stratified by age, race/ethnicity, and sex. We replicated our process in the 2016 BRFSS survey.
PARTICIPANTS
We analyzed data from 449,492 adult participants of the 2017 BRFSS survey.
MAIN MEASURES
We examined area under the curve (AUC) statistics to examine model fit within each group. We used traditional logistic regression to predict self-rated health associated with features identified by machine learning models.
KEY RESULTS
Each algorithm, regularized logistic regression (AUC: 0.81), random forest (AUC: 0.80), and support vector machine (AUC: 0.81), provided good model fit in the BRFSS. Predictors of self-rated health were similar by sex and race/ethnicity but differed by age. Socioeconomic features were prominent predictors of self-rated health in mid-life age groups. Income [OR: 1.70 (95% CI: 1.62-1.80)], education [OR: 2.02 (95% CI: 1.89, 2.16)], physical activity [OR: 1.52 (95% CI: 1.46-1.58)], depression [OR: 0.66 (95% CI: 0.63-0.68)], difficulty concentrating [OR: 0.62 (95% CI: 0.58-0.66)], and hypertension [OR: 0.59 (95% CI: 0.57-0.61)] all predicted the odds of excellent or very good self-rated health.
CONCLUSIONS
Our analysis of BRFSS data show social determinants of health are prominent predictors of self-rated health in mid-life. Our work may demonstrate promising practices for using machine learning to advance health equity.

BACKGROUND
Mental health disorders are common in youth with rheumatological disease yet optimal intervention strategies are understudied in this population. We examined patient and parent perspectives on mental health intervention for youth with rheumatological disease.
METHODS
We conducted a mixed methods cross-sectional study, via anonymous online survey, developed by researchers together with patient/parent partners, to quantitatively and qualitatively examine youth experiences with mental health services and resources in North America. Patients ages 14-24 years with juvenile idiopathic arthritis, juvenile dermatomyositis, or systemic lupus erythematous, and parents of patients ages 8-24 with these diseases were eligible (not required to participate in pairs). Participants self-reported mental health problems (categorized into clinician-diagnosed disorders vs self-diagnosed symptoms) and treatments (e.g. therapy, medications) received for the youth. Multivariate linear regression models compared patient and parent mean Likert ratings for level of: i) comfort with mental health providers, and ii) barriers to seeking mental health services, adjusting for potential confounders (patient age, gender, disease duration, and patient/parent visual analog score for disease-related health). Participants indicated usefulness of mental health resources; text responses describing these experiences were analyzed by qualitative description.
RESULTS
Participants included 123 patients and 324 parents. Patients reported clinician-diagnosed anxiety (39%) and depression (35%); another 27 and 18% endorsed self-diagnosed symptoms of these disorders, respectively. 80% of patients with clinician-diagnosed disorders reported receiving treatment, while 11% of those with self-diagnosed symptoms reported any treatment. Patients were less comfortable than parents with all mental health providers. The top two barriers to treatment for patients and parents were concerns about mental health providers not understanding the rheumatological disease, and inadequate insurance coverage. Over 60% had used patient mental health resources, and over 60% of these participants found them to be helpful, although text responses identified a desire for resources tailored to patients with rheumatological disease.
CONCLUSION
Self-reported mental health problems are prevalent for youth in this sample with rheumatological disease, and obstacles to mental health treatment include disease-related and logistic factors. Strategies are needed to improve acceptance and accessibility of mental health intervention, including routine mental health screening and availability of disease-specific mental health resources.

Pruritus is a common symptom of inflammatory skin conditions, including atopic dermatitis (AD). Although primary sensory neurons that transmit pruritic signals are well-cataloged, little is known about the neuronal alterations that occur as a result of skin disruption in AD. To address this question, we examined the molecular and behavioral consequences of challenging mice, which overexpress PAR2 in suprabasal keratinocytes, with serial topical application of the environmental allergen house dust mite (HDM). We monitored behavior and used RNA sequencing, qPCR, and in situ hybridization to evaluate gene expression in trigeminal ganglia (TG), before and after HDM. We found that neither nor wild-type (WT) mice exhibited spontaneous scratching, and pruritogen-induced acute scratching did not differ. In contrast, HDM exacerbated scratching in mice. Despite the absence of scratching in untreated mice, several TG genes in these mice were up-regulated compared to WT. HDM treatment of the mice enhanced up-regulation of this set of genes and induced additional genes, many within the subset of TG neurons that express TRPV1. The same set of genes was up-regulated in HDM-treated mice that did not scratch, but at lesser magnitude. Finally, we recorded comparable transcriptional changes in IL31Tg mice, demonstrating that a common genetic program is induced in two AD models. Taken together, we conclude that transcriptional changes that occur in primary sensory neurons in dermatitis-susceptible animals underlie a genetic priming that not only sensitizes the animal to chronic allergens but also contributes to pruritus in atopic skin disease.

BACKGROUND
Due to concerns of inadequate primary care access, national agencies like the Health Resources and Services Administration (HRSA) support primary care (PC) residencies. Recent research demonstrates that up to 35% of PC alumni lost interest in PC during residency. These alumni who lost interest noted that their continuity clinic experience influenced their career choice. The purpose of this study was to identify the specific aspects of PC residency experience that influenced career choice.
METHODS
We conducted a cross-sectional electronic survey of a PC internal medicine alumni cohort (2000-2015) from a large, academic residency. Our primary predictor was PC career and our primary outcome was influential factors on career choice. We performed chi-squared or Fisher's exact tests for categorical variables and t tests for continuous variables.
RESULTS
Of the 317 PC alumni in the last 15 years, 305 were contacted. One hundred seventy-two (56%) responded with 94 (55%) reporting current careers in PC and 78 (45%) in non-PC fields. Ninety-four percent of respondents expressed interest prior to residency, while only 68% remained interested at the conclusion of residency. Sixty-one percent of PC alumni rated the overall clinic experience as the most influential factor towards their ultimate career choice. The patient-physician relationship was the most frequently endorsed positively influential factor in career choice in both groups (95% of PC alumni, 76% non-PC). There was no difference among all alumni in common frustrations of clinic including clerical duties, encounter documentation, or visit length. Similarly, resident debt did not differ between groups.
CONCLUSIONS
Strong interpersonal relationships with patients and clinic mentors were associated with a PC career. These factors may compensate for the reported frustrations of clinic. Enhancing patient and mentor relationships may increase the retention of PC residents in ambulatory careers and may help address the current and projected shortage of primary care physicians.

Association between degenerative temporomandibular joint disorders, vertical facial growth, and airway dimension.

BACKGROUND
The goal of this study was to determine whether preexisting degenerative temporomandibular joint (TMJ) disorders are associated with hyperdivergent facial phenotype and decreased airway dimensions.
METHODS
Cone-beam computed tomography scans of adult female and male individuals, 16 years of age and older, distributed in a case group defined as those with degenerative temporomandibular joint disorder (dTMJD; n = 31) or controls with normal TMJ findings (n = 242) were included. Odds ratios were calculated based on facial type and gender. Analysis of variance was used to compare the airway volume and cross section and mandibular measurements between the groups.
RESULTS
Condylar, ramus, and mandibular heights were significantly smaller in the case group compared with the control group. The odds of having a long face subject was significantly higher (P 0.00001) in the dTMJD group than in the control group with 81% of the dTMJD subjects versus 11% of the control group having long vertical facial dimensions. The smallest cross-sectional area of the airway of the dTMJD group was significantly narrower (P 0.0361) compared with the controls. Within the control group, ramus height and mandibular alveolar housing for central incisors were significantly smaller (P 0.0001; P 0.007) in the long face subjects.
CONCLUSIONS
The study shows that a long facial type is associated with findings of degenerative TMJ disorders and related condylar growth disturbances. These degenerative and growth changes may contribute to specific skeletal and dentofacial adaptations resulting in smaller condylar process, mandibular ramus, and body height; thinner alveolar housing at the lower incisor region; and smaller cross-sectional area of the airway.
