Supervised Study Approval Form

Guidelines

The Departmental 198 Supervised Study courses for fourth-year students allow you to design a study or project that captures your interest and supplements your learning and development. These are distinct from the Department 150 Research courses that are for hypothesis-based research and from the IDS 196 course that provides credit for direct teaching or course-specific projects. The credit for Departmental 198 Supervised Study courses, which is capped at four weeks total, counts toward the discretionary graduation requirements.

To craft a Departmental 198 Supervised Study project/study, you need to:

  • Think about your topic or area of study. The scope can be as broad or narrow as you’d like, but should have the same rigor and substance as a established course.
  • Structure the project/study by outlining its learning objectives and linking them to competencies. The Departmental 198 Supervised Study application form is a good starting point when thinking about the project/study details.
  • Identify a UCSF faculty mentor who will work with you on your project design, approve your proposal, and oversee your study.

Instructions

  1. If you haven’t already activated your UCSF DocuSign account, please follow these instructions:
    • Login to MyAccess: https://myaccess.ucsf.eduhttps://myaccess.ucsf.edu
    • Scroll down to “DocuSign – UCSF” and click on the link to DocuSign. This will initiate creating your account if you don’t already have one (the account creation takes a minute or so).
    • Once signed in, you will see a status box, as well as help resources should you want more info about DocuSign.
  2. Submit the form at least 4 weeks prior to the start of the supervised study time. If the form is not submitted by this deadline, you may not be able to receive credit for your work.
  3. Select the link below for the corresponding department's supervised study elective approval form. If you need a preview of the form before initiating it with the link below, you can see this sample.
  4. Follow the instructions for completing the form. It will be sent electronically to the listed Instructor of Record.
  5. When the course is approved, you will receive an email confirmation.
  6. It is your responsibility to add the course to your study list for the appropriate term(s). Be careful to add the departmental 198 course (e.g., Medicine 198), NOT IDS 198.
